Summary of role: Social Prescribing Practitioner  with DHI is an award-winning charity that challenges social exclusion by supporting people to achieve their potential and contribute to the richness and wellbeing of their community. DHI has recently won a contract to run an innovative Social Prescribing project with Wessex Water looking at the effect of medicine waste in water. We’re looking for people who would like to get involved and do brief, light-touch empowering work with patients of 2 GP surgeries who have a range of non medical issues including benefits, debt, social isolation etc. The aim is to work with the individual to identify their needs, then refer or signpost to other organisations, services or community groups as appropriate.

Being a really good communicator and building a quick rapport in this role is essential whilst also having the ability to understand diverse and complex client needs and to use a ‘brokerage’ approach to meet these needs, if this sound like your next role then please see below on how to apply.
