Job Title: Leaving Care Worker

Summary of role: In this role you will focus on supporting care leavers in their transition to independence and achieving positive outcomes.  Key to achieving this will be through building positive and trusting relationships and  working with young people to identify their strengths and areas for development, goals and aspirations and drawing  up support and pathway plans.

Job tenure: Full time

Annual salary/pro rata salary: £21,693 to £23,866 per year NJC Scale 23 - 26, starting at the bottom of the scale
